Before
a surgical intervention, a pre-operative interview with the patient
takes
place. During this conversation the planned surgery as well as its
risks are
discussed with him in detail. Alongside the surgical procedure,
the risks of
the intervention, and the patient history, a possible incapacity
for work,
bringing an escort person and the correct behaviour after the
operation are
discussed. The preliminary conversation
should help the
patient to evaluate the necessity and the risk of the
intervention, to adjust
to it and to behave accordingly.

The apical inflammation is the
reaction in the body to the bacteria
remaining in the root channel. As a
result of it, an inflammatory tissue
(granulome) replaces bones at the
top of the root. By
means of
root-canal-preparation, the top can
not be reached. Bacteria remain in
the unfilled parts of canal.
After an incision in the gum
tissue is made and the
parts
of the bones over the affected
tips of the roots are opened,
bones as
well as tips of the
roots are removed and the
inflammatory tissue is scraped
out. After that it can be normally
closed over the root canal
(orthograd) or
from the removed
root tip (retrograd) with the
appropriate filling material.
